DOT SAP Evaluation

Service Description

Clinician provides DOT/SAP return to work evaluations and treatment to those in safety sensitive positions who fall under the jurisdiction of the Department of Transportation (D.O.T.) that received a violation due to a positive alcohol or drug test. The SAP evaluation requires a minimum of 2 hours of interview and testing time. The fee is due at the beginning of the interview session. The Department of Transportation (DOT) has mandated that certain employees are subject to federal drug and alcohol use and testing regulations. Any employee who violates these regulations must be immediately removed from safety-sensitive responsibilities, and may not be returned to any safety-sensitive duty in the transportation industry until and unless he/she successfully completes a DOT return-to-duty processes, which includes a SAP evaluation.

Cancellation Policy

Since the scheduling of an appointment involves the reservation of time set aside especially for you, a minimum of 24 hours notice is required for rescheduling or cancellation of an appointment. If for any reason a session is canceled less than 24 hours prior, the full fee will be charged.
